We conducted a pilot trial determine if novel high-resolution DWI MRI sequence (hrMRI) 18F-fluciclovine-PET-MRI could identify biopsy-proven cancers not seen on standard multiparametric (mpMRI). METHODS: Candidates for hemigland HIFU were prospectively enrolled (unilateral Gleason≥7, PSA<20, stage≤cT2c) underwent 18F-fluciclovine-PET-MRI, hrMRI mpMRI. MR/ultrasound-fusion+systematic mapping biopsy was performed, targeting lesions (PIRADS3+ or PET+) identified any imaging modality. Patients with unilateral high-grade disease partial-gland HIFU. The primary endpoint sensitivity modalities cancer. secondary control based 6-month-post-HIFU-MR/ultrasound-fusion biopsy. RESULTS: Of 20 men enrolled, 13 eligible post-HIFU analysis. showed trend toward improvement in detection over mpMRI alone. When positive test defined as that either (PIRADS 3-5) PET positive, 0.56 (95%CI,0.44–0.78) compared 0.41 (95%CI, 0.31–0.60) alone (p=0.056). For GG2+ cancers, 0.60 0.46–0.90) 0.55 0.42–0.83) (p=0.32). Five PIRADS3+ but mpM RI, which 2 GG1 cancers. Eleven mpMRI, 1 GG2 cancer, affected treatment template (Figure). High-intensity focused ultrasound (HIFU) for prostate cancer.

BACKGROUND The growing interest in high-intensity focused ultrasound (HIFU) is mainly due to its potential applications as a minimally invasive therapy. HIFU has been assessed for its role in the treatment of localized prostate cancer in patients who otherwise would not have benefited from surgery and in local recurrences after radiation failure. METHODS Relevant information on HIFU treatment...

High-intensity focused ultrasound for prostate cancer: a practice guideline.

OBJECTIVE The aim of this practice guideline was to develop evidence-based recommendations for clinicians on the use of high-intensity focused ultrasound (HIFU) in patients with localized prostate cancer. METHODS The guideline was developed using the methods of Cancer Care Ontario's Program in Evidence-Based Care (PEBC). The core methodology of the PEBC's guideline development process is syst...

High-Intensity Focused Ultrasound (HIFU) in Localized Prostate Cancer Treatment

BACKGROUND High-intensity focused ultrasound (HIFU) applies high-intensity focused ultrasound energy to locally heat and destroy diseased or damaged tissue through ablation. This study intended to review HIFU to explain the fundamentals of HIFU, evaluate the evidence concerning the role of HIFU in the treatment of prostate cancer (PC), review the technologies used to perform HIFU and the publis...

Treatment of localised prostate cancer using high-intensity focused ultrasound.

OBJECTIVE To evaluate the biochemical disease-free survival (DFS), predictors of clinical outcome and morbidity of patients with localized prostate cancer treated with high-intensity focused ultrasound (HIFU), a noninvasive treatment that induces complete coagulative necrosis of a tumour at depth through the intact skin. PATIENTS AND METHODS In all, 63 patients with stage T1c-2bN0M0 localized...
